Our Health Care Approach

Care Coordination

Our care coordinators work directly with clients to connect them with primary care physicians, specialists, and community health resources. We help eliminate barriers to care — including transportation, insurance enrollment, and appointment scheduling — so clients receive the services they need.

Health Education

We provide health education workshops and one-on-one coaching covering nutrition, chronic disease management, preventive care, and healthy lifestyle choices. Knowledge is the first step toward better health outcomes.

Benefits Navigation

Navigating Medicaid, PeachCare, Medicare, and other public health benefit programs can be overwhelming. Our staff helps clients understand their eligibility, complete applications, and maximize the benefits available to them.

Family Health Support

Healthy families start with informed caregivers. We offer family-centered health support including pediatric wellness guidance, maternal health resources, and caregiver education to help every family member thrive.
